We have made our new patient log-in available on this website for your convenience. You can create your account and complete our new patient forms at home before visiting our office. Completing your forms online will save time and help expedite your check in process and your first visit. This will also be your account to view all patient information, treatment, and financial account. Please bring medical and dental cards to your appointment as well as a drivers licence or other government issued identification (for adults only). The State of Texas requires government issued picture identification on file for all adult patients.
